The PHIT4DC program will be hosting its second PCMH Training Course this upcoming May. We strongly believe that the PCMH training is positioned to make an impact on the way patients in our community receive quality care and equity of care. The program will take 20 participants. Apply Today!

The top three benefits of PCMH are:

  1. Advances equity of care: The benefits of health equity cannot be over-emphasized. Health equity has a huge role in addressing Social Determinants of Health (SDOH) needs. Identifying available resources in the community to address these needs, such as – access to transportation, tackling food deserts, and connecting patients to community resources, adds to the overall success of health outcomes. 
  2. Increases access to healthcare: One of the direct benefits of PCMH training is the ability of the care team to practice at the top of their license. What this means is that the care team can perform roles that are within their scope of practice and do it confidently. This directly benefits the patients by increasing access to care through follow-up visits, pre-visit planning, and ensuring continuity of care that aligns with the patient’s care plan. Community members can attend their appointments through in-person visits, telehealth, and audit visits. This allows patients to access healthcare in a way that gives them a shot at great health outcomes.
  3. Making the patient the center of care– As patients begin to feel like stakeholders in their care, there is an urgent sense of health awareness and understanding of the benefits of continuity of care. Training members of the care team in our community can make a huge impact on the way patients understand the role they play in their health outcomes.

 

Howard University will play a key role in enhancing patient-centeredness by implementing and sustaining PCMH training for the community around it.
